2. Current Physical On File
In addition to the above Athletic Medical History Questionnaire, student-athletes wishing to participate in an interscholastic sport at JPII HS are required to have a CURRENT PHYSICAL from their doctor on file at the school. CURRENT PHYSICAL dates for the 2011-2012 interscholastic sport seasons are:

• Fall sports: Physicals on or after July 22, 2010;
• Winter sports: Physicals on or after October 28, 2010;
• Spring sports: Physicals on or after February 19, 2011.

Please contact your physician for a copy. Student-athletes will not be allowed to participate until both the Athletic Medical History Questionnaire and a physician's physical are on file.
